ActivOT respects our clients’ right to privacy. As Occupational Therapists (OTs) we are bound by our professional code of conduct. This includes how we treat the information received from or about our clients.
As health service providers, we are also bound by the National Privacy Principles.
In order to provide a high quality of service we need to obtain information about our clients including personal particulars. This information may include, but is not necessarily limited to, the following:
- Names
- Date of birth
- Contact details (address, telephone numbers etc)
- Current and historical medical information
- Reason for referral
- Next of kin/emergency contact details
This information is used to provide the best possible OT service to our clients. If this information is not provided, we may not be able to provide effective services to our clients. The information is stored securely and for a time period prescribed by Australian legislation. After this time all records are securely disposed of or deleted.
Clients have the right to access any of their personal information that is collected and stored by ActivOT .
ActivOT will not disclose any information obtained about clients without consent, except where particular laws require its disclosure or where ActivOT has mandatory reporting obligations. In the event of an emergency where there may be a serious threat to the life or health of any person emergency services will be contacted.
With the client’s consent we will:
- provide feedback to a client’s GP/LMO &/or referrer
- information to home modification service providers and equipment suppliers in order for ActivOT to achieve outcomes as agreed by the client.
When an individual is unable to provide consent to the collection, use and disclosure of their personal information ActivOT will obtain permission from their guardian or carer.
ActivOT will ensure that client privacy is protected at all times in accordance with the Privacy Act 1988.
